Cardholder's legible signature, faxed or mailed, is required for all orders. Orders from established accounts do not require a faxed signature sheet. Establishing an account is simple: When you place your first order, fill out and fax or mail the Transmission Form with completed information. Your sheet with signature will be kept on file for processing future orders. If you change billing address you must complete and send a new form.
